You know our patients are much more than just customers as a Social Services Specialist at Dycora. Partnering with the our professional and collaborative staff, you'll help our patients reach their full potential during their stay at our facilities and after discharge. Your job is vital to making sure our patients and their families are as comfortable as possible and are treated with the utmost respect during challenging times. Are you ready to take a whole life into your hands and help patients get back on their own two feet? The right person will not take no for an answer when it comes to our patient's comfort, happiness and well-being. Ideal Social Services Specialist candidates should also have:

  • Develop a comprehensive social history assessment, a psycho-social assessment, discharge plan, and complete the MDS and Care Plan for new admits. Orient and assist new residents in adjusting to the facility. Assist residents and families with adjustment to changes, including room changes. Assist families with the engraving and/or marking of residents' personal items. Review residents' rights with residents and staff, and attend to various personal needs of the residents. Assist in the development of a volunteer program and assist in facilitating family group meetings. Document progress notes that relate to each resident's care plan when necessary and within policy timeframe

  • Comply with all Company policies related to safety and infection control procedures including the proper use of mechanical lifts, gait belts and personal protective back supports

  • Understand, comply with and promote all rules regarding resident rights, promote positive relationships with residents, visitors and regulators, to include presenting a professional appearance

  • Other duties as assigned or needed

Requirements

  • Associate's Degree in Social Work or related field. Two (2) years equivalent experience in social services position in lieu of degree

  • Healthcare experience preferred

  • Must meet any state and/or Federal requirements if applicable

  • Basic understanding of computer technology, including email use

  • Must be capable of maintaining regular attendance
