CDS Life Transitions Manager of Nursing Services in Webster, New York

Summary:

Under supervision of the Director of Nursing Services, coordinates daily activities of Registered Nurses and Licensed Practical Nurses. Provides nursing services to assigned sites, consultation, and any required clinical supervision related to the provision of nursing services. Function as an Interdisciplinary Treatment Team (ITT) member and assure compliance with all applicable standards. Under direction of supervisor, provides and coordinates training to nurses and audits assigned sites as scheduled.

Essential Job Functions:

  • Provides direct clinical supervision to RN’s, LPNs, and other professional clinicians as required. Assists in completion of performance measures and participates in decision making for employment and termination of RN’s, LPN’s, and clinicians.

  • Assists in ensuring that Initial and Annual Nursing Assessments are provided on all individuals, following the format established by the program.

  • Assists in ensuring that annual/bi-annual medication reviews are provided for all program participants.

  • Provides nursing services on a regular basis to achieve individual goals and services to CDS Monarch residences, Day Service locations, and other sites as assigned.

  • Trains the administration of prescribed medications to individuals, review of physicians’ orders, and maintenance of sanitary conditions to nursing staff.

  • Assists in coordinating the provision of information and referral to medical or mental health providers, and other clinical providers as needed.

  • Participate in Initial, Annual, and Quarterly Reviews and clinical meetings as needed by supervisor. Serve on assigned committees (IRC, HRC, MERC and Safety and Wellness Committees)

  • Assists in maintaining all required documents per Agency policy and procedures.

  • Provides or coordinates required education and training to staff including medication certification, new employee orientation, and specific nursing needs.

  • Adhere to all Agency and Program policies and procedures. Assist in maintenance and updating all nursing policy and procedures as needed to ensure they are current at all times.

  • Interview and recommend employees for hire, following Agency policy and procedures as needed

  • Ensures all new nurses are trained on Agency policy and procedures.

  • Completes regular nursing audits.

  • Coordinate and schedule regular and as needed meetings with RN’s and LPN’s as needed

  • Perform all other necessary duties relevant to the position as requested by the supervisor.

  • Advise direct supervisor of any obstacles to achieving responsibilities.

    Knowledge, Skills, and Abilities

  • Ability to work independently and motivate others.

  • Ability to communicate effectively, both orally and in writing.

  • Proficient in time-management skills

    Education and Experience:

  • Must be licensed and currently registered as a Registered Nurse by the State of New York State Education Department.

  • Specialized training or one year of professional experience in treating those with developmental disabilities.

  • Must be at least 18 years of age.

  • Reliable transportation (NYS Drivers’ Licensed required).

    Physical Requirements/Working Conditions:

  • Ability to sit continuously

  • Ability to reach above shoulder level

  • Ability to turn/twist upper body

  • Ability to use hand for repetitive action and fine manipulating for the purpose of keyboarding

    The listed salary range represents the organization's good faith and reasonable estimate of the range of possible compensation at the time of posting.
